Registered Nurse (RN) – Home Care
Location: Bala Cynwyd, PA 

Type: local contract
Shift/hours: 8:00am-4:30pm, 40 hours/week 

Job Summary:
The Home Care Registered Nurse (RN) provides skilled nursing care to patients in their homes, promoting independence, comfort, and optimal health outcomes. The RN works collaboratively with physicians, caregivers, and interdisciplinary teams to develop and implement individualized care plans.

Key Responsibilities:
Conduct comprehensive patient assessments, including physical, emotional, and environmental needs
Develop, implement, and update individualized care plans
Administer medications, treatments, and wound care as prescribed
Monitor patient conditions and document changes accurately
Educate patients and families on disease management, medications, and home care techniques
Coordinate care with physicians, therapists, and other healthcare professionals
Ensure compliance with healthcare regulations and agency policies
Provide end-of-life care and support when applicable
Maintain accurate and timely clinical documentation
 

Qualifications:
Active Registered Nurse (RN) license
Graduate of an accredited nursing program (BSN preferred)
Current CPR/BLS certification
Valid driver's license and reliable transportation
Previous home care or clinical nursing experience preferred
Skills & Competencies:
