Qualifications

Active LPN/LVN license in the state where services will be provided.

Graduate of an accredited practical nursing program.

Minimum 1 year of Home Health experience within the past 3 years (required).

Current CPR/BLS certification.

HCHB (Homecare Homebase) EMR experience required.

Reliable personal transportation required.

Strong clinical, communication, and documentation skills.

Key Responsibilities

Provide skilled nursing care and monitor patients' health status during home visits.

Administer medications and treatments within the scope of practice and physician orders.

Observe and report changes in patients' conditions to the supervising RN or physician.

Educate patients and caregivers on medications, treatment plans, and disease management.

Maintain accurate and timely documentation using HCHB EMR.

Coordinate care with RNs, physicians, and other healthcare professionals.

Follow applicable state regulations and Home Health standards.
