Qualifications

Active SLP license in the state where services will be provided.

Master's degree or equivalent qualification from an accredited Speech-Language Pathology program.

Minimum 1 year of Home Health experience within the past 3 years (required).

Current CPR/BLS certification.

HCHB (Homecare Homebase) EMR experience required.

Reliable personal transportation required.

Strong clinical, communication, and documentation skills.

Key Responsibilities

Evaluate patients' speech, language, cognitive-communication, and swallowing abilities.

Develop and implement individualized treatment plans based on patient needs.

Provide therapy to improve communication, cognitive function, and swallowing safety.

Educate patients and caregivers on therapeutic strategies and home exercise programs.

Document evaluations, progress notes, and treatment outcomes using HCHB EMR.

Coordinate care with physicians and the interdisciplinary team.

Follow applicable state regulations and Home Health standards.
