Description
Elder Life Management, now part of the BAYADA family, is seeking a compassionate, adaptable, and clinically strong Registered Nurse (RN) Care Manager to support older adults and their families in Tinton Falls, NJ with clients primarily in central jersey.
This is a unique opportunity for an RN who wants to use their clinical judgment in a more independent, relationship-driven role. As an RN Care Manager, you will work directly with older adults and their families to assess needs, develop individualized plans, coordinate services, advocate across settings, and help clients remain as safe, supported, and independent as possible.
This role is ideal for a nurse who enjoys meaningful one-on-one relationships, problem-solving, home visits, and guiding families through complex and changing situations. It is not a shift-based bedside role, and it is not a traditional office-based clinical manager role. It is a community-based nursing role that requires flexibility, maturity, sound judgment, and the ability to pivot as client needs change.

What you’ll do
As an RN Care Manager, you will:
- Conduct comprehensive assessments of each client’s physical, psychological, environmental, developmental, and support needs
- Evaluate the client’s current status and develop an individualized plan of care based on assessment findings
- Use nursing judgment to identify risks, changes in condition, unmet needs, and appropriate next steps
- Coordinate home care services and other service systems to support safety, well-being, and quality of life
- Conduct home visits and support clients in their own homes or other selected environments
- Serve as an advocate for clients and families as they navigate aging, changing health needs, and care decisions
- Review care management plans regularly and adjust services as client needs evolve
- Support client autonomy while guiding appropriate service utilization
- Maintain accurate documentation of client status, care coordination activity, and ongoing service needs
- Coordinate placement when needed, including identifying appropriate facilities, supporting the application and acceptance process, and helping with transition planning
- Ensure important planning conversations and documentation are addressed, including advance directives, power of attorney, wills, and burial plans, as appropriate

What you bring
- Current Registered Nurse (RN) license in good standing
- Four-year college degree
- Comfort with home visits and working in a community-based role
- Ability to communicate effectively with clients, families, providers, and community partners
- Strong organizational skills and timely documentation habits
- Confidence working independently and managing a changing daily schedule
- Demonstrated ability to take on responsibility, adapt, and continue learning
- Basic PC skills required to perform job functions
Preferred background
- Geriatrics, elder care, home care, care management, case management, hospice, rehabilitation, assisted living, or community-based nursing experience
- Experience supporting families through transitions, placement decisions, or long-term planning
- Experience coordinating care across multiple service providers or settings
Schedule and work environment
- Full-time (Monday- Friday 8:30-5pm)
- Field-based work in Tinton Falls and Central Jersey with flexibility needed based on client needs
- Home visits, care coordination, family communication, and documentation are all core parts of the role
- Agency car provided for work-related travel
