Qualifications

Required: - Current licensure in New York State - Minimum of two (2) years of experience in a home setting within the last ten (10) years - Recent experience conducting comprehensive assessments of patients - Experience in: - Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC) - Conducting assessments similar to UAS, such as OASIS - Primary care to homebound individuals - Care management for home and community-based services, including Hospice and Assisted Living - Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse, caring for a minimum of 2 patients per week

Preferred: - Experience conducting assessments in UAS-NY (role 40 or 45), especially in the last 365 days - Experience with geriatric population and managing chronic diseases, especially in the home setting - Combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of 2 patients per week

Other: - Experience that may count towards preferred qualifications but not required: - Home care experience with one patient (e.g., private duty nursing) - Case management limited to utilization reviews - Telephonic assessments without in-home visits

Job Responsibilities

Conduct initial in-home evaluations of patients, assessing health status, and care needs to guide long-term care service plans.

Review relevant consumer medical documentation or health referral forms.

Enter evaluation data into electronic forms and transmit as required.

Document concerns, conflicting information, and issues during evaluations.

Emphasize continuity of care to reduce fragmentation and duplication in treatment plans.

Discuss healthcare options, necessary supports, service vendor options, and waiver options with consumers.

Facilitate communication between the Call Center and eligible consumers for health plan enrollment.

Enhance collaboration with interdisciplinary care team to improve care coordination.

Collect quality review data and required documentation to support outcomes and record case notes into CRM.

Identify opportunities for health promotion and illness prevention.

Maintain a comprehensive knowledge of community resources, payor requirements, and network services for the target population.

Meet all standards outlined in the corresponding annual performance criteria for the position.

Perform additional duties as assigned by project management.
