Qualifications
Required: - Current licensure in New York State - Minimum of two (2) years of experience in a home setting within the last ten (10) years - Recent experience conducting comprehensive assessments of patients - Experience in: - Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC) - Conducting assessments similar to UAS, such as OASIS - Primary care to homebound individuals - Care management for home and community-based services, including Hospice and Assisted Living - Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse, caring for a minimum of 2 patients per week
Preferred: - Experience conducting assessments in UAS-NY (role 40 or 45), especially in the last 365 days - Experience with geriatric population and managing chronic diseases, especially in the home setting - Combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of 2 patients per week
Other: - Experience that may count towards preferred qualifications but not required: - Home care experience with one patient (e.g., private duty nursing) - Case management limited to utilization reviews - Telephonic assessments without in-home visits
