Eligibility Requirements:
- Arizona residents are not eligible for assignments in Arizona facilities.
- Candidates must be flexible with locations, shifts, and departments to expedite securing an offer.
Assignment Conditions:
- Candidates must be ready to start assignments.
- Requests for time off (RTO) must not exceed 7 days and cannot occur during the first week of the assignment.
Submission Requirements:
- Minimum of 24 months of Oncology experience, with recent experience in the last 12 months.
- Life support certifications must be from AHA, ARC, or ASHI.
- State license must have no hits or restrictions.
- Valid driver’s license required to verify residency.
- Flu vaccination is required, with medical and/or religious exemption forms available upon request. Include vaccination status in submission notes.
- Travel experience is highly preferred.
Compliance and Documentation:
- Adherence to background check standards is mandatory.
- Complete administrative paperwork, such as the cover page, as required.
- A resume should list all facilities, departments worked, trauma level designations, and the number of beds.
- An Oncology skills checklist must be completed within the last 12 months.
- Active license to practice verified within the last 30 days.
- Active BLS certification from an approved vendor.
- Active ONS/ONCC Chemotherapy Immunotherapy Certification.
- Flu vaccination documentation.
- Copy of the driver's license.
Certifications Required:
- ACLS (AHA)
- BLS (AHA)
- Chemotherapy/Biotherapy Infusion Certification
Note: On-call and weekend details will be determined by the facility.
