Eligibility Requirements:

• Arizona residents are not eligible for assignments in Arizona facilities.
• Candidates must be flexible with locations, shifts, and departments to expedite securing an offer.

Assignment Conditions:

• Candidates must be ready to start assignments.
• Requests for time off (RTO) must not exceed 7 days and cannot occur during the first week of the assignment.

Submission Requirements:

• Minimum of 24 months of Oncology experience, with recent experience in the last 12 months.
• Life support certifications must be from AHA, ARC, or ASHI.
• State license must have no hits or restrictions.
• Valid driver’s license required to verify residency.
• Flu vaccination is required, with medical and/or religious exemption forms available upon request. Include vaccination status in submission notes.
• Travel experience is highly preferred.

Compliance and Documentation:

• Adherence to background check standards is mandatory.
• Complete administrative paperwork, such as the cover page, as required.
• A resume should list all facilities, departments worked, trauma level designations, and the number of beds.
• An Oncology skills checklist must be completed within the last 12 months.
• Active license to practice verified within the last 30 days.
• Active BLS certification from an approved vendor.
• Active ONS/ONCC Chemotherapy Immunotherapy Certification.
• Flu vaccination documentation.
• Copy of the driver's license.

Certifications Required:

• ACLS (AHA)
• BLS (AHA)
• Chemotherapy/Biotherapy Infusion Certification

Note: On-call and weekend details will be determined by the facility.
