Overview
The Post-Lung Transplant Coordinators are integral members of the lung transplant program team, reporting to the Medical Director and under administrative oversight from the Transplant Center Director or their designee. Their main responsibility is to ensure the continuity of care for patients post-discharge from transplantation. They act as the central reference point for information about patients in the program, overseeing facets like education, quality assurance, research, administrative coordination, and financial activities.
Principal Duties and Responsibilities
Patient Care Post-Transplant Discharge Phase:
- Deliver care to achieve optimal physical, social, and emotional rehabilitation within the designated scope of practice and licensure.
- Identify and address the educational, psychosocial, and economic needs of recipients, family members, and legal guardians in the post-transplant phase.
- Reinforce education and follow-up requirements crucial for a successful outcome.
- Monitor compliance with medications and medical regimens, collaborating with the transplant team to maintain it.
- Collect and review data relevant to the recipient's health and organ function, ensuring accurate documentation in the patient's chart.
- Coordinate lab work and tests for post-transplant clinic visits, and communicate lab data and diagnostic results to the transplant physician or healthcare provider.
- Implement changes in drug or treatment therapy as directed by physicians and other credentialed healthcare providers.
- Understand post-transplant protocols and assist internal and external healthcare providers.
- Arrange consultations, diagnostic procedures, infusions, and hospitalizations as needed.
- Maintain communication with referring physicians, primary care providers, and other healthcare personnel.
- Ensure comprehensive documentation is maintained in the patient's medical record.
