The physical therapist is responsible for providing physical therapy services in a home health setting. The primary function involves collaborating with other team members and participating in the development of the patient’s comprehensive plan of care. Rehabilitation equipment used includes splints, orthoses, prostheses, wheelchairs, and ADL equipment modalities.

Requirements:

• 2+ years of acute nursing care (hospital base) and home care experience required.
• Valid driver's license required.
• Must be able to work independently with minimal supervision.
• Certified home health experience, including proficiency with OASIS, is required.
• Must have own vehicle.

Profile Requirements:

• Work history and skills checklist for a physical therapist must be current to within one year.
• One supervisory signed reference from the past two years of employment.
• Massachusetts Physical Therapist License must be included in the profile.
• Active American Heart Association BLS certification required.
