Complete Description:
Experience Required:
- Must have Oasis experience
- Minimum of one year experience in Long Term Care/Home Care setting as a Physical Therapist
- Recent Home Care experience (within the last year)
Qualifications:
- A B.S., M.S. or D.P.T. degree in Physical Therapy
- Current New York State License in Physical Therapy
- BLS (AHA ONLY)
- Good verbal and written communication skills
- Good interpersonal skills
- Ability to perform physical therapy procedures effectively and safely
- Ability to work with people from different socio-cultural backgrounds effectively
- Ability to physically lift and assist patients in hands-on care
- Ability to accept and utilize supervision
Relationships:
Works closely with patients, families, physicians, hospital staff, vendors, community agencies, third-party payors, care delivery team members, and various departments within the organization.
Responsibilities:
- Provides case management and direct patient care within a team environment
- Assists the physician with evaluating level of function and helps develop the plan of care
- Prepares clinical and progress notes
- Participates and assures delivery services align with the agency vision, mission, values, and strategic goals
- Promotes effective working relationships and teamwork
- Utilizes resources, supplies, and time efficiently
- Ensures practices, standards, and behaviors meet agency performance competencies
- Maintains productivity requirements and participates in performance improvement processes
Direct Patient Care Duties:
- Develops and revises treatment plans and discharge plans
- Initiates appropriate rehabilitative program
- Provides supervision to PTAs, ensuring patient health, safety, and plan compliance
- Consults with and educates the patient, family, and team members
- Evaluates patient's progress and adjusts the physical therapy program as needed
- Instructs para-professional staff in various techniques
- Directs, coordinates, evaluates, and supervises quality of patient care services provided
- Supports care management philosophy, including collaborative planning and case management

Key Skills:
- Home Health Experience within the last year
- 1 year of PT experience in a home health setting
- NYS PT License
- BLS (AHA ONLY)
