Job Description

Requirements:

• Master's Degree and LMSW, LCSW, LMHC, or LCAT license required.
• Familiarity with NYS OMH requirements is necessary, along with prior New York work history.
• Experience with individuals aged 5 and above.

Responsibilities:

• Provide crisis intervention and counseling in both outpatient and emergency department settings.
• Conduct comprehensive assessments, gathering data on psychosocial history, behavioral observations, substance dependence/abuse, and more.
• Develop clinical formulation as the framework for treatment planning.
• Establish treatment goals and interventions in collaboration with clients and families.
• Ensure timely and accurate documentation within the clinic's electronic medical records.
• Participate in on-call rotations for mental health crisis evaluations.

Assessment and Treatment Planning:

• Gather and analyze comprehensive assessment data, including the risk of harm and support systems.
• Screen for co-occurring substance use disorders.
• Formulate treatment plans that are person-centered, recovery-oriented, and culturally responsive.
• Develop individualized and evidence-based treatment strategies with clearly defined goals and objectives.
• Update and revise treatment plans regularly, including discharge criteria and progress evaluations.

Documentation and Quality Improvement:

• Maintain thorough clinic records and progress notes linked to treatment goals and client responses.
• Utilize evidence-based interventions to support healthy behaviors and symptom management.
• Systematically evaluate the quality and effectiveness of therapeutic practices.
• Engage in self-evaluation to identify strengths and areas for professional development.

Collaboration and Engagement:

• Work collaboratively with clinic team members to meet productivity standards and improve client engagement.
• Participate in quality improvement activities to enhance service delivery within the clinic.

Skills:

• Basic Life Support (BLS) certification required.
• Minimum of 2 years of experience as an LCSW or LMSW.
• Covid and flu vaccination required.
