Description

Essential Functions (Key Roles & Responsibilities)

Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues.
Formulate biopsychosocial assessment, disposition, and treatment plans.
Collaborate with other professionals to evaluate patients' medical or physical conditions and assess client needs as part of the interdisciplinary team.
Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Perform other duties as assigned.
Comply with all policies and standards.

Education

Master's Degree in Mental Health & Behavioral Medicine required, or
Master's Degree in Social Work required.

Class D Passenger Vehicle Driver's License [State License] preferred
Licensed Clinical Social Worker [LCSW - State License] preferred
Licensed Independent Clinical Social Worker [LICSW - State License] preferred
