Description

Essential Functions

  • Systematically gather and evaluate relevant patient information from interviews and other sources.
  • Ensure patient exams, tests, and procedures align with the patient's needs and orders.
  • Identify variations from expected findings and inform the charge nurse when necessary.
  • Provide patient teachings and involve family, significant others, or friends in the educational process when appropriate.
  • Prioritize requests based on their nature and the patient's condition.
  • Assess and reassess patient conditions as required.
  • Coordinate and communicate with healthcare professionals to deliver comprehensive patient services.
  • Delegate tasks appropriately to other healthcare professionals.
  • Collaborate with departmental and healthcare team members to complete assignments.
  • Support comprehensive case management to assure patient success and compliance with standards of care.
  • Collect patient information from various sources to create a comprehensive database and initial treatment plan.
  • Coordinate care with the primary care provider, outpatient providers, and other services.
  • Participate in multidisciplinary treatment planning to align care with patient needs.
  • Respond to assigned case management tasks for diagnostics, treatment planning, and disposition.
  • Verify insurance details and obtain necessary approvals or referrals for services provided.
  • Offer individual and group therapies following Mental Health Unit standards.
  • Document provided interventions and patient responses according to established policies.
  • Support Mental Health Unit staff in supervising students and interns to uphold performance and documentation standards.
  • Engage in professional growth activities to enhance knowledge, skills, and abilities, sharing insights with the team.

Job Requirements

  • Minimum Education: Bachelor's degree required in Psychology, Human Services, or a related field. Master's degree preferred.
  • Minimum Work Experience: At least 1 year of clinical experience in the Human Services field. Demonstrated ability to interact effectively with diverse patients and caregivers.
  • Licenses and Certifications: AHA BLS required or obtained within 90 days of hire.
