Position Description

• Perform comprehensive nursing assessment upon patient admission, considering physiological, psychological, and environmental factors, as well as health history, to ensure safety and quality of care.
• Integrate pertinent data from assessments to identify problems for the plan of care, adhering to case management objectives.
• Utilize HHRG values to establish visit frequency and interdisciplinary utilization.
• Ensure annual outcomes meet National Benchmarks and refer to community resources as needed.
• Evaluate clinical needs and develop, implement, and revise the patient's plan of care, addressing all problems and documenting accordingly.
• Participate in interdisciplinary patient case conferences to evaluate the effectiveness of the plan of care.
• Instruct and document supervision of the Home Health Aide plan of care every 14 days.
• Notify physicians and other care providers of significant changes in the patient's condition.
• Complete admission and other necessary paperwork promptly.
• Record narrative documentation to reflect skilled care, ensuring clarity and conciseness in addressing specifics of teaching, direct care, and response from patients or caregivers.
• Obtain and clarify valid medical orders for all care delivered.
• Plan and coordinate discharges appropriately, inform the patient and physician, and record patient disposition in the discharge summary.
• Complete all necessary discharge paperwork.
• Perform all other duties as assigned.

Qualifications

• Successful passage of pre-hire medication test.
• Current RN license in Massachusetts and related credentials to meet specialty requirements.
• Minimum one year of nursing experience required, or six months with a BSN.
• CPR certification required.
• Home care, IV experience, computer skills, and BSN preferred.
