Coordinates and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Degree of Travel:
- Home visits and periodic office meetings. - Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Patient Information:
- Average patients per day: 5-8 - Points per week: 30. Each visit and activity (documentation, travel, etc.) holds a different point value. Assignments typically involve 5-10 patients per day, equating to roughly 6-8 points.

Guaranteed Hours:
- No guarantee. If unable to meet financial needs due to low patient census, discussion is encouraged about floating to nearby clinics.

Types of Cases:
- Start of Care, Wound Care, Follow-up Visits, etc.

Experience Required:
- Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on specialty. - First-timers not accepted; recent specialty experience is required.

Certifications Required:
- CPR, BLS

Territory and Coverage:
- Must be willing to travel up to 75 miles from the home clinic. - Territory mileage radius includes 150-200 miles per week travel expectations.

Dress Code:
- Scrubs

Charting Requirements:
- Proficiency with HomeCare HomeBase (HCHB) is required and must have been used within the last 6 months. - RNs, PTs, and OTs must be proficient in Oasis.

Schedule:
- Contract assignment typically lasts 13 weeks, with standard schedule Monday to Friday, 8 am to 5 pm. - As needed, weekend rotations and on-call responsibilities may arise.
