Qualifications:

1 year of certified home care experience
1 year of case management experience
Current licensure in the state of Massachusetts
Reliable, independent means of transportation for home visits
Willingness to work weekends, holidays, and on-call as needed, with weekend rotations every six weeks

Responsibilities:

Provide clinical services to patients in their homes, ranging from birth to over 100 years old
Assess patient conditions and implement care plans
Conduct admissions and manage cases according to policies and standards
Ensure thorough and timely electronic documentation

Care will be provided in the following areas: - Abington - Bridgewater - Brockton - Carver - Cohasset - Duxbury - East Bridgewater - Halifax - Hanover - Hanson (Monponsett) - Hanson - Hingham - Hull - Kingston - Lakeville - Marshfield (Green Harbor) - Marshfield (Humarock) - Marshfield - Middleboro - Norwell - Onset - Pembroke (Bryantville) - Pembroke - Plymouth - Plympton - Rockland - Scituate (GreenBush) - Scituate (Minot) - Scituate - Wareham - West Bridgewater - Weymouth
