Description

Requirements:

Must hold a Physical Therapist Professional license in Massachusetts.
Minimum of 1 year home care experience required.
Must be able to travel to assigned cases.

Responsibilities:

Provide physical therapy services to patients in their place of residence with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
Utilize thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and procedures.

Service areas include Beverly, Marblehead, Salem, and Swampscott.
