Description

Requirements:

  • Must have recent Level III or Level IV NICU experience.
  • Arizona residents are not eligible for assignments in any Arizona facility.
  • Must have a minimum of 24 months NICU experience, with recent experience in the last 12 months.
  • Life support certifications must be from AHA, ARC, or ASHI.
  • No issues on state license to practice.
  • RTO cannot exceed 7 days and is not allowed during the first week of the assignment.
  • Valid driver's license to verify residency.
  • Flu vaccination is required; medical and/or religious exemption forms are available upon request.
  • Travel experience is highly preferred.
  • Strict adherence to background check standards.

Profile Requirements:

  • Complete the designated cover page.
  • Submit a detailed resume, including all facilities, departments worked, trauma level designation, and number of beds.
  • NICU skills checklist must be completed within the last 12 months.
  • Active license to practice must be verified within the last 30 days.
  • Active BLS certification from an approved vendor.
  • Active NRP certification from an approved vendor.
  • Documentation for flu vaccination is required.

Certifications:

  • NRP
  • BLS (AHA)

  • Candidates must be open about the locations, shifts, and departments they are willing to accept for quicker placement.
  • Must be ready to accept an offer upon submission.
  • Immediate notification required if a candidate withdraws or is no longer available.
  • Indicate all facilities, states, or clearly state readiness to work in any location.
  • Specify all shifts and departments the candidate is experienced and willing to work in.

  • Weekend and on-call requirements will be determined by the facility.
