Description

Work Location:
45 Reade Place, Poughkeepsie, NY 12601

Responsibilities:
- Social worker with case management experience in an acute care environment and in transitions of care/discharge planning.
- Internal floating within the case management team is required to ensure coverage of all inpatient and outpatient areas.
- Perform discharge planning services such as:
- Skilled nursing facility placement
- Home care coordination
- Durable medical equipment arrangements
- Transportation coordination
- Insurance authorization support
- Family communication

Required Qualifications:
- NYS LCSW or LMSW
- 3 years of acute care hospital discharge planning experience
- Cerner experience
- WellSky experience

Preferred Qualifications:
- Experience with RoundTrip, Parachute, and Payor portals
