Description

Required Qualifications

Current RN Licensure in New York State
Two (2) years of experience in a home setting within the last ten (10) years
Recent experience conducting comprehensive assessments of patients in home settings, such as:
Certified Home Health Agency (CHHA)
Licensed Home Care Service Agency (LHCSA)
Managed Long-Term Care (MLTC)
Experience conducting assessments similar to OASIS

Preferred Qualifications

Experience conducting assessments in UAS-NY (role 40 or 45) within the last year
Experience caring for a geriatric population and managing chronic diseases in a home setting
Combination of home care experience as a paraprofessional or LPN and RN, caring for at least 2 patients per week

Other Relevant Experience

Home care experience with one patient (e.g., private duty nursing)
Case management limited to utilization reviews for a health plan
Telephonic assessments without home visits

Job Summary

Review health status questions and make evaluations based on UAS criteria
Work with technology in a professional setting
Excellent communication and organizational skills
Ability to work independently and handle a fast-paced environment

Job Responsibilities

Conduct initial in-home pre-evaluation of patients to assess health status, strengths, and care needs

Review relevant medical documentation and health referral forms

Enter data into electronic evaluation forms and transmit as required

Document concerns and issues during the evaluation process

Ensure continuity of care to prevent fragmentation and duplication

Discuss healthcare options and services with consumers

Communicate with Call Center and eligible consumers for health plan enrollment

Enhance coordination with care team members to improve service delivery

Collect quality review data and support outcome measurements

Identify opportunities for health promotion and illness prevention

Maintain knowledge of community resources, payor requirements, and services

Meet standards outlined in annual performance criteria

Perform other duties as assigned by project management
