Required Qualifications:
- Current licensure in New York State.
- Two years of experience in a home setting within the last ten years.
- Recent experience conducting comprehensive assessments of patients.
Preferred Qualifications:
- Experience conducting assessments in UAS-NY, especially in the last year.
- Experience caring for a geriatric population and managing chronic diseases in the home setting.
- Combination of home care experience as a paraprofessional or LPN caring for a minimum of 2 patients per week.
Other Relevant Experience:
- Home care experience with private duty nursing and Consumer Directed Personal Assistance Services.
- Case management limited to utilization reviews for a health plan or a combination of hospital and health plan case management.
- Assessments conducted telephonically without home visits.

Job Responsibilities:
Perform initial in-home evaluations of patients to assess health status, strengths, care needs, and preferences, guiding the development of individualized long-term care service plans.
Review consumer medical documentation and health referral forms, as relevant.
Enter evaluation data into electronic forms and transmit as required.
Document concerns, conflicting information, and other issues during the evaluation process.
Emphasize continuity of care to reduce fragmentation, duplication, and gaps in treatment plans.
Discuss healthcare options, supports needed, and service vendor options with consumers.
Facilitate communication between the Call Center and eligible consumers for health plan enrollment.
Enhance collaboration with interdisciplinary care team members to improve care coordination and service delivery.
Collect quality review data and required documentation to support outcome measurements and record case notes into CRM.
Identify opportunities for health promotion and illness prevention.
Maintain comprehensive knowledge of community resources, payer requirements, and network services for the target population.
Meet all established standards for this position.
Perform other duties as assigned by the Regional Nurse Manager or project management.
