Job Summary:
Deliver physical therapy services to patients at their place of residence who experience physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may vary in age from birth to over 100 years.
Responsibilities:
- Collaborate with patients, their families, physicians, and other healthcare professionals to identify patient needs and coordinate necessary services.
- Utilize thorough and timely electronic documentation.
- Engage in interdisciplinary communications following agency policies, procedures, and standards of practice.
