Essential Duties and Responsibilities

• Perform evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider, while observing precautions, indications, and contraindications.
• Evaluate residents per department protocols and establish a therapeutic plan of care, including functional long- and short-term goals, frequency and duration of services, and therapeutic modalities.
• Obtain input from residents and primary caregivers regarding the therapeutic plan of care and incorporate this information into the therapy goals.
• Provide consultation to the referring physician and other healthcare team members regarding the plan of care and discharge needs of the resident.
• Monitor and evaluate the treatment effect, progressing the treatment program toward functional goals established in the treatment plan of care. Modify the treatment plan as necessary.
• Document evaluative and therapeutic interactions with residents in the Electronic Medical Record per department procedures. Ensure documentation is accessible and easily understood.
• Recommend and procure appropriate assistive devices for residents, ensuring instruction in their use.
• Provide appropriate education to residents and/or caregivers and document the results of teaching.
• Refer residents appropriately to Physical Therapist Assistants and work collaboratively with other therapy professionals.
• Understand and comply with Medicare and other reimbursement regulations.
• Provide in-service education regarding clinical knowledge and skills to nursing and other departments, maintaining current clinical skills and knowledge.
• Assist with the development of new Rehabilitation programs and services.
• Stay informed on professional issues through membership in professional organizations and reading professional publications.
• Supervise the activities of assistant-level staff, ensuring adherence to state mandates and standards.
• Provide mentorship to new staff during orientation.
• Maintain cleanliness and maintenance of department areas, following infection control procedures.
• Maintain required licenses, certifications, competencies, and complete annual compliance courses on time.

Education and Experience

• Bachelor's degree required; Master's or Doctorate preferred
• Active license as a PT in the applicable State of work
• Basic Life Support (BLS) certification is preferred
• Prior experience in a long-term care or skilled nursing facility preferred
• An equivalent combination of education and experience may be substituted for the above requirements

Working Conditions/Physical Demands

• Must possess the physical and mental abilities to perform tasks involving sitting, standing, walking, bending, stooping, twisting, and reaching.
• Must be able to lift up to 50 pounds. Lifting above 50 pounds requires a two-person lift.
• Frequently lift/carry up to 20 pounds.
• Some stress is related to a high level of responsibility for quality care.
