Essential Duties and Responsibilities
- Perform evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider, while observing precautions, indications, and contraindications.
- Evaluate residents per department protocols and establish a therapeutic plan of care, including functional long- and short-term goals, frequency and duration of services, and therapeutic modalities.
- Obtain input from residents and primary caregivers regarding the therapeutic plan of care and incorporate this information into the therapy goals.
- Provide consultation to the referring physician and other healthcare team members regarding the plan of care and discharge needs of the resident.
- Monitor and evaluate the treatment effect, progressing the treatment program toward functional goals established in the treatment plan of care. Modify the treatment plan as necessary.
- Document evaluative and therapeutic interactions with residents in the Electronic Medical Record per department procedures. Ensure documentation is accessible and easily understood.
- Recommend and procure appropriate assistive devices for residents, ensuring instruction in their use.
- Provide appropriate education to residents and/or caregivers and document the results of teaching.
- Refer residents appropriately to Physical Therapist Assistants and work collaboratively with other therapy professionals.
- Understand and comply with Medicare and other reimbursement regulations.
- Provide in-service education regarding clinical knowledge and skills to nursing and other departments, maintaining current clinical skills and knowledge.
- Assist with the development of new Rehabilitation programs and services.
- Stay informed on professional issues through membership in professional organizations and reading professional publications.
- Supervise the activities of assistant-level staff, ensuring adherence to state mandates and standards.
- Provide mentorship to new staff during orientation.
- Maintain cleanliness and maintenance of department areas, following infection control procedures.
- Maintain required licenses, certifications, competencies, and complete annual compliance courses on time.

Education and Experience
- Bachelor's degree required; Master's or Doctorate preferred
- Active license as a PT in the applicable State of work
- Basic Life Support (BLS) certification is preferred
- Prior experience in a long-term care or skilled nursing facility preferred
- An equivalent combination of education and experience may be substituted for the above requirements
Working Conditions/Physical Demands
- Must possess the physical and mental abilities to perform tasks involving sitting, standing, walking, bending, stooping, twisting, and reaching.
- Must be able to lift up to 50 pounds. Lifting above 50 pounds requires a two-person lift.
- Frequently lift/carry up to 20 pounds.
- Some stress is related to a high level of responsibility for quality care.
