Summary

The Physical Therapist partners with residents and other care providers to offer physical therapy services to maximize functional abilities and independence in activities of daily living.

Essential Duties and Responsibilities

  • Perform evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider while observing precautions, indications, and contraindications.
  • Evaluate residents per department protocols and establish a therapeutic plan of care, including functional long- and short-term goals, frequency and duration of services, and therapeutic modalities to use.
  • Seek input from the resident and primary caregivers regarding the therapeutic plan of care and incorporate this information into therapy goals.
  • Provide consultation to referring physicians and other members of the healthcare team regarding the plan of care and discharge needs of the resident. Attend all pertinent team meetings.
  • Monitor and evaluate the treatment effect and progress treatment programs toward the functional goals established in the plan of care. Modify the treatment plan as necessary.
  • Document evaluative and therapeutic interactions in the Electronic Medical Record per department procedures, ensuring accessibility and understandable language for other care team members.
  • Recommend and obtain appropriate assistive devices for the resident and ensure instruction is given for their use.
  • Provide appropriate education to residents and/or caregivers and document the results of teaching.
  • Refer residents appropriately to Physical Therapist Assistants and work collaboratively with other therapy professionals for resident care.
  • Understand and comply with Medicare and other reimbursement regulations.
  • Document resident care appropriately, with attention to reimbursement requirements.
  • Provide in-service education regarding clinical knowledge and skills to nursing and other departments, and maintain current clinical skills and knowledge through learning activities.
  • Actively assist with developing new programs and services offered by Rehabilitation.
  • Maintain awareness of professional issues through membership in professional organizations and reading professional publications. Bring pertinent matters to staff attention.
  • Supervise assistant-level staff activities, considering their level of education and training. Ensure adherence to state mandates and professional organization standards.
  • Provide mentorship to new staff during orientation, ensuring that departmental procedures are followed.
  • Ensure department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat. Follow all system and department infection control procedures.
  • Maintain required licenses, certifications, and competencies, and complete annual compliance courses on time.

Education and Experience

  • Bachelor’s degree required. Master’s or Doctorate preferred.
  • Basic Life Support (BLS) certification is preferred
  • Prior experience in a long-term care or skilled nursing facility preferred

An equivalent combination of education and experience that provides proficiency in the areas of responsibility listed above may be substituted for the education and experience requirements.

Working Conditions/Physical Demands

  • Must possess the physical and mental abilities to perform tasks normally associated with this position, including sitting, standing, walking, bending, stooping, twisting, and reaching.
  • Must be able to lift up to 50 pounds. Lifting above 50 pounds requires a two-person lift.
  • Lifting/carrying frequently up to 20 pounds.
  • Some stress related to high-level responsibility for quality care.
