Licensure and Experience Requirements

Current licensure in New York State
Two years of experience in a home setting within the last fifteen years
Recent experience conducting comprehensive assessments of patients

Job Summary

Candidates must be capable of reviewing health status questions, making evaluations, and determinations based on specific criteria for enrollment. Must be proficient with technology and possess knowledge of the theory, principles, and practices of general professional nursing, as well as state and agency laws and regulations. Strong organizational, interpersonal, written, and verbal communication skills are essential, including the ability to engage with multi-generational consumers and demonstrate sensitivity to others' concerns. Candidates must be able to thrive in fast-paced, deadline-oriented environments and execute multiple complex tasks simultaneously while working independently.

Job Responsibilities

Perform initial in-home pre-evaluation of patients, including evaluation of health status, strengths, care needs, and preferences to guide long-term care service plans.

Review consumer medical documentation or health referral forms pertinent to the case.

Enter evaluation data into electronic evaluation forms and transmit as required.

Document any concerns, conflicting information, or issues identified during evaluations.

Emphasize continuity of care to reduce or eliminate fragmentation and gaps in treatment plans.

Discuss healthcare options, necessary supports, service vendor options, and waiver options with consumers.

Facilitate communication between the Call Center and eligible consumers for enrollment in a managed care health plan.

Enhance communication and collaboration with interdisciplinary care team members to improve care coordination and facilitate service delivery.

Collect quality review data and document case notes into CRM to support outcome measurements.

Identify opportunities for health promotion and illness prevention.

Maintain a comprehensive knowledge of community resources, payor requirements, and network services for the target population.

Meet all standards established for the position, as outlined in the annual performance criteria.

Perform other duties as assigned by the Regional Nurse Manager or project management.
