Description

Required Licensure

  • Current licensure in New York State

Required Experience

  • Two years of experience in a home setting within the last ten years, which may include:
  • Working in the field for a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC)
  • Conducting assessments resembling UAS such as OASIS
  • Providing primary care to homebound persons
  • Care management for individuals receiving home and community-based services including Hospice and Assisted Living
  • Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse caring for a minimum of two patients per week

Preferred Experience

  • Experience conducting assessments in UAS-NY (role 40 or 45), particularly in the last 365 days
  • Experience with a geriatric population and managing their chronic diseases in a home setting
  • Combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of two patients per week

Other Experience

Examples that may count toward preferred qualifications but do not satisfy the required qualifications:

  • Home care experience with only one patient (e.g., private duty nursing and Consumer Directed Personal Assistance Services)
  • Case management limited to utilization reviews for a health plan or a combination of hospital and health plan case management
  • Telephonic assessments without home visits

Job Summary

  • Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment
  • Proficiency with workplace technology
  • Knowledge of the theory, principles, and practices of professional nursing
  • Familiarity with state and agency laws and regulations governing nursing practices
  • Excellent organizational, interpersonal, written, and verbal communication skills
  • Ability to effectively communicate and work with multi-generational consumers
  • Sensitivity to others' concerns
  • Capacity to perform in a fast-paced, deadline-oriented environment
  • Strong ability to work independently

Job Responsibilities

  • Conduct initial in-home pre-evaluation of patients to assess health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans
  • Review consumer medical documentation and health referral forms, as relevant
  • Enter evaluation data into electronic evaluation form and submit as required
  • Document any concerns, conflicting information, or other issues during evaluation
  • Emphasize continuity of care to reduce fragmentation and gaps in treatment plans
  • Discuss healthcare options, supports needed, service vendor options, and waiver options with the consumer
  • Initiate communication for enrollment into a managed care health plan
  • Enhance communication with interdisciplinary care team members to improve care coordination
  • Collect quality review data and documentation to support outcome measurements and record case notes into CRM
  • Identify opportunities for health promotion and illness prevention
  • Maintain comprehensive knowledge of community resources, payor requirements, and network services for the target population
  • Meet all performance standards for this position
  • Perform other duties as assigned by the Regional Nurse Manager or project management
