Compensation
Weekly Gross Pay: $3,400/week

Overtime Pay: $95/hour (after 40 hours)

Job Summary

We are seeking an experienced Physical Therapist (PT) to provide physical therapy services in a Skilled Nursing Facility (SNF) setting.

The PT will work with residents and care providers to maximize functional abilities, promote independence in activities of daily living, and develop individualized plans of care based on each resident's needs.

Key Responsibilities
Evaluate residents and establish individualized physical therapy plans of care.
Perform therapeutic procedures under appropriate medical orders and supervision requirements.
Establish short- and long-term functional goals.
Provide therapeutic interventions and monitor treatment outcomes.
Modify treatment plans based on resident progress and clinical needs.
Collaborate with residents, caregivers, physicians, nursing staff, and other healthcare professionals.
Provide education to residents and caregivers regarding treatment plans and assistive devices.
Refer residents to Physical Therapist Assistants when appropriate and supervise assistant-level staff as required.
Maintain accurate and timely documentation in the Electronic Medical Record.
Ensure documentation meets Medicare, Medicaid, private-pay, and other reimbursement requirements.
Participate in interdisciplinary team meetings and contribute to discharge planning.
Maintain current clinical knowledge, licenses, certifications, and competencies.
Follow infection control, safety, and facility procedures.
Provide mentorship and support to new staff when appropriate.
Requirements
Active Physical Therapist (PT) license – Required
Minimum 3 years of experience – Required
Previous Skilled Nursing Facility experience – Required
Bachelor's degree – Required
Master's or Doctorate preferred
BLS/BCLS – Required
Strong clinical, communication, documentation, and time-management skills
Ability to work collaboratively within an interdisciplinary team
Proficiency with Microsoft Office
Ability to provide compassionate, resident-centered care
Ability to meet the physical demands of the position, including standing, walking, bending, lifting, and carrying
