Role: Registered Nurse (RN) – Home Health
Location: St. Johnsbury, Vermont (VT)
Facility Name: Northern Counties Health Care, Inc.
Contract Duration: 13 Weeks
Shift Timing: Days, Monday–Friday (Possible Weekend Shifts)
Scheduled Hours: 36 Hours/Week (Guaranteed)
Required Experience:
• Minimum 1 year of clinical nursing experience preferred
• Home Health experience preferred

Certifications & Mandatory Skills:
• Active Vermont RN License
• BLS Certification
• Strong patient assessment, care planning, and case management skills
• Experience with Homecare Homebase (HCHB) preferred
• Ability to provide admissions, evaluations, and individualized care plan development

• RN will provide comprehensive home health nursing services to patients of all ages.
• Responsibilities include admissions, evaluations, care planning, coordination of care, and ongoing patient assessments.
• Work collaboratively with an interdisciplinary home health team to ensure quality patient outcomes.
• Coverage area includes Caledonia County and Southern Essex County, Vermont.
• Possible weekend shifts based on patient care needs.
• No on-call requirement.
• Guaranteed 36 hours per week.
• Facility uses Homecare Homebase (HCHB) EMR.
• Facility does not accept 1099 contractors.
• Maximum of 3 candidate submissions accepted.
