We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team at Autumn Lake at Scotch Plains. The ideal candidate will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, function, and overall quality of life. Responsibilities include assessing patients, establishing and implementing treatment plans, providing physical therapy services in accordance with physician orders and facility policies, monitoring patient progress, modifying treatment plans as needed, educating patients, families, and caregivers on therapeutic exercises and mobility techniques, maintaining accurate and timely documentation, and collaborating with interdisciplinary healthcare teams to achieve optimal patient outcomes. Candidates must possess an active Physical Therapist (PT) license in the State of New Jersey and strong communication and patient care skills; current BLS certification may be required by the facility. Previous experience in skilled nursing, rehabilitation, or long-term care settings is preferred. To be considered, applicants must submit an updated resume and complete the Stern Pre-Screen.
