Locations: Lebanon, NH area White River Junction, VT area Visiting Nurse and Hospice for VT & NH covers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. When you join Visiting Nurse and Hospice for VT & NH, you’ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve. Hospice RN Case Managers at VNH focus on providing pain management and comfort for patients with terminal or chronic illness, as well as communicate and coordinate care needs with the interdisciplinary team, the patient and their families/caregivers. The RN Case Manager - Hospice: Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate. Coordinates the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Provides ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and VNH colleagues. Documents evaluations, patient response, and progress towards goals. Teaches the patient and caregiver care techniques and end-of-life symptom management as appropriate. Minimum Qualifications: Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license. Graduate of an accredited school of nursing. Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
