Home Health Social Workers offer care and assistance to individuals during their home health journeys, as well as their families and close ones. Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting everyday life challenges. Home Health social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

Job Duties

  • Provide individual and family counseling in the home setting.
  • Assist with establishing advanced directive and durable power of attorney.
  • Facilitate the utilization of community and home care resources.
  • Attend and participate in IDG.
  • Make referrals to long-term care facilities as needed.
  • Develop and facilitate support groups.
  • Meet with other Social Workers and Hospice team.
  • Maintain professional relationship with staff and referral sources.

Requirements

  • Masters-in-Social Work or Licensed Clinical Social Worker (LCSW)
  • Home Health experience preferred.
  • Ability to work in a professional manner.
  • Must work well both independently and in a group setting.
  • Professional appearance
  • Excellent attendance and punctuality
  • Problem solving skills and ability to multi-task.
  • Excellent Communication Skills
  • Positive, Professional attitude
