Job Summary
Provide physical therapy services in the home under the direction of a licensed PT
Follow the physician plan of care to support patient progress
Communicate with the care team to ensure quality and safety
Use clinical judgment to deliver effective treatments
Maintain accurate and timely documentation
Follow all safety, compliance, and infection control standards

Requirements
Valid PTA license in Kansas or Missouri
Home health experience preferred but not required
Ability to work independently and as part of a team
Strong communication and time management skills
Ability to pass background check, drug screen, and physical
