Description

"Duties
- Assess patients and develop, evaluate, and update discharge plans.
- Coordinate care and services to ensure appropriate patient placement.
- Monitor patient progress, length of stay, and barriers to discharge.
- Conduct utilization reviews and assess medical necessity.
- Apply admission criteria and support continued-stay reviews.
- Follow Medicare regulations, including patient choice, IMM notices, and MOON notices.
- Coordinate with interdisciplinary teams to facilitate timely discharges.

Qualifications & Certifications
- License: Active Georgia RN or Compact RN License required at submission.
- Experience: Minimum 3 years of RN experience.
- Travel Experience: Required; first-time travelers not accepted.
- Certification: BLS required.
- Charting: Experience with any electronic charting system required.
- Preferred: Teaching hospital and community hospital experience."
