In addition, this position serves as Case Manager in cases in which this position is the lead discipline.COVID VACCINATION REQUIRED - no exemptions.Oasis experience is a must.Home health experience is required.Experience as a home care traveler with proficiency in OASIS is preferred, as this role includes case management responsibilities.Homecare Home Base experience is preferred.
