Minimum Requirements:

• Pharmacy License for OH or Compact as applicable
• Applicable recent experience in this work setting
• Minimum Experience varies by Profession and setting.
• Commitment to quality care and adherence to client legal and client protocols
• Certifications: Must have a valid Ohio Pharmacy License in good standing with the Ohio Board of Pharmacy
• Equipment/Procedures: hospital experience; operates as the sole Pharmacist in the hospital from 1130 - 0600, with CPhT support
• Weekend/Call: Weekend = Required; Call = None stated
• Experience: 1 year
