• Allied Health License for AK or Compact as applicable
• Applicable recent experience in this Allied Health Physical Therapist work setting
• Minimum Experience varies by Profession and setting.

• Certifications: Licensed as a Physical Therapist; Current Basic Life Support certification; Valid Driver's license
• Experience: 1 year
• Local Restrictions: Travelers allowed
