Description
Active, unrestricted NY RN license – MUST
ASN/Associate’s in Nursing – MUST
3+ years nursing experience
2+ years Case Management / Discharge Planning / Home Healthcare Coordination
Medicare and/or Medicaid experience
Health assessment & care planning experience
Chronic medical conditions, SDoH, and/or behavioral health experience
Must live in an EST/East Coast time zone
Remote/WFH experience and dedicated workspace
