Job Requirements
- Education Requirements: Bachelor’s degree in Physical Therapy from an accredited institution
- Certifications Requirements: NBCOT certification, Montana PT license, BLS certification
- Experience Requirements: Minimum of 1 year of PT experience, preferably in SNF settings; experience with PDPM and Section GG preferred
- Other Requirements: Familiarity with NetHealth / PointClickCare EMR system; strong interpersonal skills; ability to work independently and manage time effectively

Job Responsibilities
- Provide skilled physical therapy assessments and develop individualized treatment plans for patients in a rehab setting.
- Collaborate with healthcare team members to coordinate patient care and ensure the achievement of therapy goals.
- Maintain accurate documentation of patient progress and therapy sessions using NetHealth / PointClickCare EMR.
- Execute therapy interventions and modify treatment plans based on patient response and progress.
- Educate patients and their families about post-treatment care, exercises, and injury prevention strategies.
