Connected Health Care is seeking a Local Travel Physical Therapist (PT) for a 13-week contract assignment with one of our trusted Skilled Nursing Facility (SNF) partners in Helena, MT. This opportunity offers competitive weekly compensation up to $2,550, consistent 8-hour day shifts, and the chance to work in a supportive rehab environment focused on patient-centered care.

Position Overview – Physical Therapist (PT)
The Physical Therapist (PT) will evaluate, develop, and implement individualized treatment plans for patients in a Skilled Nursing Facility. This role focuses on restoring function, improving mobility, reducing pain, and preventing long-term disability through evidence-based physical therapy interventions.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Provide therapeutic exercises, manual therapy, gait training, and mobility interventions
- Collaborate with physicians, occupational therapists, speech therapists, and nursing staff
- Educate patients and families on rehabilitation goals and home exercise programs
- Document patient progress and maintain accurate EMR records
- Ensure compliance with federal, state, and facility regulations
- Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate
- Active Montana Physical Therapist (PT) License
- Experience working in a Skilled Nursing Facility (SNF)
- Strong clinical, documentation, and communication skills
- Ability to work independently and within an interdisciplinary team
- Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy from an accredited program
- Successful completion of the National Physical Therapy Examination (NPTE)
- Active Montana PT License in good standing
- Current BLS Certification preferred
