• Education Requirements: Valid Registered Nursing (RN) license in the state of Illinois
• Certifications Requirements:
  • BLS certification
  • EPIC EMR experience preferred
  • Additional relevant certifications are a plus
• Experience Requirements:
  • At least 1 year of hospice care experience as an RN
  • Experience with community-based patient visits
  • Familiarity with high-acuity patient management
• Other Requirements:
  • Ability to troubleshoot medical devices such as pleurx drains and nephrostomy tubes
  • Strong communication and patient/family education skills
  • Must be N95 fit tested prior to arrival
