Job Requirements- Education Requirements: Valid Registered Nursing (RN) license in the state of Minnesota
- Certifications Requirements: Basic Life Support (BLS) certification, CPR certification, Healthcare Provider certification
- Experience Requirements: Minimum of 1 year of experience as a Registered Nurse, prior experience in long-term care or skilled nursing facilities, experience coordinating MDS assessments
- Other Requirements: Strong communication and organizational skills, ability to work independently, proficiency with Electronic Medical Records (EMR) systems

- Provide comprehensive MDS assessments and coordinate the MDS process to ensure accurate and timely completion in compliance with regulatory standards.
- Collaborate with interdisciplinary team members to develop and implement individualized patient care plans aligned with assessment findings.
- Maintain detailed documentation of assessments, interventions, and patient progress within the EMR system.
- Execute quality assurance protocols to ensure all MDS items are completed accurately and in accordance with federal and state regulations.
- Support staff training and education regarding MDS processes, documentation, and compliance requirements.
