Travel Licensed Practical Nurse (LPN) – LTC – 13-Week Contract – Near Savanna, IL – Up to $2,000/Week

Connected Health Care, a leading recruitment and staffing firm, is seeking an experienced Travel Licensed Practical Nurse (LPN) for an excellent opportunity with a long-term care (LTC) facility near Savanna, Illinois. Position Details

Position: Travel Licensed Practical Nurse (LPN)
Setting: Long-Term Care (LTC)
Location: Near Savanna, Illinois
Shift Options: Day & Night Shifts Available
Schedule: 3x12s (36 hours/week)
Contract Length: 13 Weeks (with potential for extension)
Start Date: ASAP
Compensation: Up to $2,000/week

Key Responsibilities

• Provide direct patient care to residents in a long-term care environment

• Administer medications and treatments in accordance with physician orders and scope of practice

• Monitor patient conditions and report changes to the RN and healthcare team

• Assist in developing and implementing individualized care plans

• Collaborate with CNAs and interdisciplinary staff to ensure quality care and continuity

• Maintain accurate and timely documentation per facility and regulatory guidelines

• Support a safe, respectful, and compassionate environment for residents

Education & Certification Requirements

• Active Licensed Practical Nurse (LPN) license (Illinois or compact eligible, if applicable)

• BLS (Basic Life Support) certification required

• Minimum 1–2 years of nursing experience, preferably in LTC, SNF, or rehab settings

• Strong clinical skills and attention to detail

• Ability to work both independently and as part of a team
