Registered Nurse (RN) or Licensed Practical Nurse (LPN) – Senior Living Community

Position Summary
Connected Health is seeking a compassionate and reliable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support one of our partner facilities, a 56-bed senior living community in Des Moines, Iowa. This role involves providing high-quality, resident-focused care while supporting the daily clinical operations of the nursing team.

Under the direction of the Director of Nursing, Nurse Supervisor, and facility leadership, you will help ensure that all care is delivered in accordance with federal, state, and local regulations, while promoting a safe, respectful, and supportive environment for residents.

Key Responsibilities

• Deliver direct nursing care to residents based on individualized care plans
• Support and collaborate with Nursing Assistants and other care team members
• For RNs: assist in supervising LPNs and Nursing Assistants and overseeing daily clinical activities
• Monitor resident conditions and report changes to the appropriate team members
• Maintain accurate documentation and ensure compliance with all regulations and facility policies
• Foster a positive and compassionate atmosphere for residents and staff

Qualifications

• Active RN or LPN license in the state of Iowa, or currently working toward licensure
• Must be at least 18 years of age
• Ability to read, write, and communicate clearly in English
• Working knowledge of nursing practices, procedures, and applicable healthcare regulations
• Previous experience in long-term care or a supervisory role is preferred, but not required
• Familiarity with regulations governing senior living or long-term care settings is a plus
• Genuine compassion and a positive attitude toward caring for the elderly
