Local Travel Physical Therapist (PT) - South of San Jose, CA
Contract Length: 13 Weeks
Weekly Compensation: Up to $3,160 (Based on Experience)

Connected Health Care is seeking an experienced Physical Therapist (PT) for a local travel assignment in San Jose, California. This is a great opportunity to work with a reputable healthcare facility while staying close to home. Whether you're a seasoned traveler or new to contract work, we provide unmatched support every step of the way.

Job Description:

Evaluate, plan, and implement individualized treatment plans for patients
Use evidence-based practices to restore function, relieve pain, and improve mobility
Educate patients and families on home exercise programs and prevention strategies
Collaborate with physicians, nursing staff, and other therapists to ensure quality care
Document patient progress and update treatment plans accordingly
Maintain compliance with all local, state, and federal regulations

Education & Certification Requirements:

Degree in Physical Therapy from an accredited program – Required
Current California PT License – Required
BLS Certification – Required
Minimum of 1 year of clinical experience preferred
