Local Travel Home Health Physical Therapist (PT) – 13 Week Contract
Independence, Missouri | Servicing Kansas & Missouri (Greater Kansas City Metro Area)

Weekly Compensation: Up to $2,400 (based on experience)
Contract Length: 13 Weeks
Connected Health Care is seeking an experienced and highly efficient Home Health Physical Therapist (PT) for a 13-week local travel assignment based in Independence, MO, with service coverage throughout Kansas and Missouri, including the greater Kansas City metro area. This opportunity is with one of our trusted healthcare partners and is ideal for clinicians who are confident working independently while delivering exceptional patient care in a home health setting.
This role requires a strong background in home health, including OASIS documentation, Start of Care visits, and case management. Candidates must be comfortable managing their own schedules and patient caseload with minimal supervision while traveling throughout the assigned service region.
Job Responsibilities
- Provide skilled physical therapy services to patients in their homes across KS and MO service areas
- Travel throughout the greater Kansas City metro region to meet patient care needs
- Perform comprehensive patient evaluations and develop individualized care plans
- Complete Start of Care assessments and ongoing patient visits
- Accurately perform and document OASIS assessments
- Manage re-certifications and case management responsibilities
- Maintain timely and compliant documentation using Matrix Care EMR
- Educate patients and caregivers on treatment plans and home exercise programs
- Monitor patient progress and modify treatment plans as needed
- Work independently while maintaining high standards of patient care
Education & Certification Requirements
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Active Physical Therapist license in Missouri and Kansas required
- Current CPR/BLS certification
- Minimum of 2 years of home health experience preferred
- Proven experience with OASIS documentation required
- Experience using Matrix Care EMR strongly preferred
