Description

Title:  RN - Clinic
Location:  Manitowoc, Wisconsin
Contract: 12 weeks
Schedule: 28.5 hours per week - 30 min breaks daily. 3 Shifts per week | TUESDAYS 645A-415P HARBOR TOWN WEDNESDAYS 645A-415P HARBOR TOWN FRIDAYS 645A-415P SHEBOYGAN N TAYLOR DRIVE | CLINIC EXPERIENCE, IN OFFICE PROCEDURE ASSIST, TELEPHONE TRIAGE, PATIENT EDUCATION No call, no weekends

Job Summary:
This position focuses on health care for individuals, families, groups, communities and populations which occurs in primary and specialty care outpatient clinics, addressing patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with other health care professionals while ensuring patient advocacy, coordination of services, implementation of nursing care, continuity of care, and patient education when needed.

Job Responsibilities:
-Systematically collects focused data relating to the health needs and concerns of a patient, group, or population while assessing physical condition including pain level related to the chief complaint and documenting subjective and objective data accurately.
-Accurately completes flow sheets for each encounter, reviews and clarifies medications, and documents details in the patient’s medication profile and EHR system.
-Completes appropriate patient assessments during triage and synthesizes available data, information, and nursing knowledge relevant to health situations and anticipated needs.
-Analyzes assessment data to determine diagnostic statements for health promotion, maintenance, and health-related issues while monitoring lab and diagnostic results daily.
-Communicates abnormal lab results to ordering providers and ensures results are directed to available providers when necessary while ensuring all required reports and referrals are ready at the time of appointment.
-Coordinates the delivery of care within and across health care settings while promoting individual and community wellness.
-Ensures safe patient care in compliance with applicable nurse practice standards and follows JACHO and HIPAA guidelines in all care delivery processes.
-Responds effectively to emergency situations and actively participates in mock drills and emergency preparedness activities.
-Facilitates communication and coordination across multiple care levels, specialties, and organizations while serving as a key point of contact for patient care coordination.
-Utilizes appropriate teaching methods tailored to patient demographics including age, gender, cultural background, and diversity considerations.
-Supports patients and caregivers in developing skills to promote and maintain health including lifestyle improvements, preventative care, and daily living activities.
-Evaluates patient progress toward goals and assesses the effectiveness and appropriateness of care provided while communicating urgent concerns to providers.
-Participates in organizational initiatives to maintain a safe, confidential, and patient-friendly environment.
-Engages in clinic committees, professional organizations, continuing education, and community programs while attending unit meetings and contributing to performance improvement projects.
-Functions as a preceptor or charge nurse when required and evaluates personal nursing practice in alignment with professional standards, regulations, and patient needs.
-Maintains clinical competencies through ongoing education, performance reviews, credential verification, and updates to procedures, equipment, and regulatory requirements.

Required Skills & Experience:
-Excellent communication skills with the ability to effectively interact with patients, caregivers, and healthcare professionals.
-Strong organizational skills to manage multiple responsibilities, documentation, and patient care coordination efficiently.

Preferred Skills & Experience:
-One (1) year of experience in medical or surgical nursing practice.

Required Education:
-Graduate of an accredited school of nursing.

Required Certifications & Licensure:
-Current State of Wisconsin Registered Nurse License.
OR
-Multi-state Nursing License from a participating state in the Nurse Licensure Compact (NLC).
-American Heart Association Basic Life Support for Healthcare Provider (BLS).
OR
-American Heart Association approved equivalent certification.
