We are seeking a dedicated and experienced Physical Therapist (PT) for a Home Health Therapy/Rehabilitation position in Accomac, VA. This contract role offers the opportunity to provide essential rehabilitation services in a home health setting.

Key Responsibilities:

• Deliver high-quality physical therapy services to patients in their homes.
• Conduct thorough assessments and develop individualized treatment plans.
• Implement therapeutic interventions to enhance patient mobility and quality of life.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive care.

Requirements:

• Minimum of 6 months to 1 year of recent home health experience.
• OASIS experience is required.
• Valid Physical Therapist license in the state of Virginia.
• Strong communication and interpersonal skills.
