Join a leading healthcare provider as a Speech and Language Pathologist (SLP) in Purcell, OK, specializing in Long Term Care (LTC) and Skilled Nursing Facility (SNF) settings. This contract position offers the opportunity to make a significant impact on the lives of patients dealing with communication, cognitive, or swallowing disorders.

In this role, you will:

• Effectively screen, evaluate, and rehabilitate patients, developing appropriate plans of care while adhering to all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practices to support clinical interventions.
• Assess patient progress and adjust treatments accordingly, documenting all required information in the clinical record as per regulatory standards.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders.
• Communicate effectively with supervisors and other health team members about patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
• Document all required information in the electronic documentation system and maintain patient records according to company policy.
• Accurately record treatment minutes for services provided in line with regulatory and clinical requirements.
• Participate in required meetings, including patient care conferences and rehabilitation meetings.
• Engage in facility and company-required trainings, in-services, and conferences.
• Obtain necessary Durable Medical Equipment (DME) or supplies as per regulatory requirements.
• Supervise and support Clinical Fellowship Year (CFY) or Student Speech-Language Pathologists.
• Adhere to ASHA and State Required practice acts, code of conduct, and ethics during professional practice as a Speech-Language Pathologist.
• Maintain active ASHA and State Licensure.
