GLC Group is seeking a qualified Physical Therapist for a contract position in the San Ramon, CA area. This role involves providing rehabilitation services across a designated county territory that includes Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point. Additional travel outside the area may be required, and weekend coverage may also be necessary.

The position offers a guarantee of 36 hours per week, with the expectation that travelers will complete at least 27 points in the week. Points are accrued based on the types of visits performed, with specific visit types converting to points and hours as outlined below:

• OASIS ReCertification and OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours.
• OASIS SoC = 1.5 points = 2 hours.

Payment is made on a per-visit basis, which includes drive/travel time, visit time, documentation time, and time spent making phone calls to patients and doctors. If the traveler refuses patients, the guarantee does not apply. Please note that the guarantee does not apply for the first and last week of the assignment, or for weeks with holidays or time off.

Credentials required for this position include a current California Physical Therapy License, BLS certification, a valid driver's license, auto insurance, auto registration, annual TB test, MMR, Varicella, HepB, TDap (within 10 years), annual flu shot, drug screen, and physical examination.

It is important that your agency's contract mirrors the confirmation page, including identical start and end dates. All requested time off (RTO) must be disclosed at the time of submission; any changes post-confirmation may result in removal from the vendor set.
