GLC Group is seeking a qualified Physical Therapist for a contract position in the San Ramon area, specifically covering the territories of Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington. Additional travel outside the area may be required, and weekend coverage may also be necessary.

If guaranteed hours apply, the following conditions will be in effect: a productivity requirement of at least 27 points per week, with a 36-hour guarantee. Travelers must complete 27 points in the week and be willing to travel outside the area or work weekends to maintain weekly visit expectations. Please note that if a traveler refuses patients, the guarantee does not apply. The guarantee is also not applicable for the first and last week of the assignment, weeks with holidays, or weeks where the traveler has time off.

For non-guarantee assignments, to secure a full caseload, travelers may be required to go outside the service area occasionally and can refuse patients. There are no guaranteed hours, and travelers must be willing to see Covid+ and Covid PUI patients. Payment is made on a per-visit basis.

Visit types and their point conversions are as follows:

• OASIS ReCertification and OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours.
• OASIS SoC = 1.5 points = 2 hours.

Although travelers are paid hourly by their agency, hours are approved by the facility on a per-visit basis. The rate is all-inclusive and covers drive/travel time, visit time, documentation time, and time spent making phone calls to patients and doctors.

• Current CA PT License
• BLS Certification
• Driver's License
• Auto Insurance
• Auto Registration
• Annual TB Test
• MMR, Varicella, HepB, TDap (10 years), Flu (annual)
• Drug Screen
• Physical Examination

Your agency's contract with the traveler must mirror the confirmation page. If your agency does not comply with this standard, it risks removal from the vendor set. The start and end dates must be identical. All requested time off (RTO) must be disclosed at the time of submission. Time off requested after the acceptance date will not be approved.
