We are seeking a dedicated Home Health Physical Therapist to join our team in Lewistown, MT. This is a contract position that offers the opportunity to make a meaningful impact on the lives of patients in a home health setting.

Key Responsibilities:

• Receive and review physician therapy orders for Home Health and Hospice patients.
• Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals.
• Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation.
• Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes.
• Maintain accurate and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation.
• Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care.
• Travel to patient homes and other care settings to provide therapy services.

Qualifications:

• Must have current PT licensure in the state of Montana.
• Must have a valid driver's license.
• BLS Certified.
